In July 2025, a tower crane operator on a high-rise project in Charlotte was blind-picking a load of steel decking from a laydown area behind an adjacent building. The designated signal person had stepped away to take a phone call, and a rigger on the ground attempted to guide the load using informal gestures the operator didn't recognize. The load struck a column, swung unpredictably, and a worker on the receiving deck was struck and critically injured. OSHA's investigation cited two violations: no qualified signal person present during a blind lift, and failure to use standard hand signals as required under Subpart CC.
Signal person failures contribute to a significant percentage of crane accidents. According to OSHA fatality investigation reports, communication breakdowns between operators and ground crews are a contributing factor in roughly 25% of crane-related incidents. When Is a Signal Person Required?
Under 29 CFR 1926.1419(a), a signal person is required whenever the point of operation — where the load is being picked up or set — is not in full view of the operator, or whenever the equipment is traveling and the operator's view in the direction of travel is obstructed. Additionally, a signal person is required whenever the operator or the person handling the load determines that a signal person is needed due to site conditions.
In practice, signal persons are needed on most construction sites for:
- Blind picks: Load is behind a building, wall, or other obstruction
- Steel erection: Ironworkers at height need to guide loads into precise positions
- Concrete pours: Bucket placement requires ground-level coordination
- Mechanical/electrical sets: Setting rooftop units, generators, or transformers in confined areas
- Night operations: Reduced visibility even with lighting
- Multi-crane lifts: Each crane typically needs its own signal person
OSHA Qualification Requirements
OSHA's signal person qualification requirements are defined in 29 CFR 1926.1428. Unlike crane operator certification — which requires third-party testing through accredited bodies — signal person qualification can be achieved through two pathways.
Pathway 1: Third-Party Qualified Evaluator
A signal person can be qualified by a third-party qualified evaluator. This is similar to crane operator certification — an independent organization assesses the signal person's knowledge and practical abilities. Organizations like the National Commission for the Certification of Crane Operators (NCCCO) offer signal person certification programs that satisfy this pathway. The NCCCO Signalperson certification includes both a written exam and a practical exam.
Pathway 2: Employer-Qualified
Alternatively, the employer can qualify signal persons internally using a “qualified evaluator.” Under 1926.1428(c), the employer's qualified evaluator must assess whether the signal person meets the following criteria:
- Knows and understands the type of signals used (hand, voice, or electronic)
- Is competent in using the signals
- Understands the operations and limitations of the equipment
- Demonstrates the ability to position themselves safely while clearly communicating with the operator
- Meets the basic requirements of 1926.1419-1926.1422 (signal person conduct standards)
The qualified evaluator conducting the assessment does not need to be a third party, but they must have the knowledge, training, and experience to determine if the signal person candidate is competent. OSHA expects documentation of this evaluation, including the evaluator's qualifications and the date and results of the assessment.
Requalification
OSHA does not specify a fixed requalification interval for signal persons. However, under 1926.1428(d), if a signal person's performance is deficient, the signal person must be re-evaluated and re-qualified before being allowed to resume signaling duties. Best practice is to reassess signal persons annually and whenever they transfer to a different type of crane operation or signaling method.
Standard Hand Signals (ASME B30.5)
Under 29 CFR 1926.1419(b), unless voice or electronic signals are used, hand signals must follow either the Standard Method defined in ASME/ANSI B30.5 or an alternative method — but if an alternative method is used, it must be agreed upon in advance and understood by all parties involved in the lift.
The ASME B30.5 standard hand signals are the universal language of crane operations in the United States. Every signal person, operator, and rigging crew member should know them without hesitation:
Primary Movement Signals
- Hoist (raise load): Forearm vertical, index finger pointing up, rotate hand in small horizontal circles
- Lower: Arm extended downward, index finger pointing down, rotate hand in small horizontal circles
- Boom Up (raise boom): Arm extended, fingers closed, thumb pointing upward
- Boom Down (lower boom): Arm extended, fingers closed, thumb pointing downward
- Swing: Arm extended, point with index finger in the direction of swing
- Stop: Arm extended, palm down, move hand right and left horizontally
- Emergency Stop: Both arms extended, palms down, move arms right and left horizontally
Travel and Telescoping Signals
- Travel (crawler/mobile): Arm extended forward, hand open and slightly raised, push in direction of travel
- Travel (one track — crawler): Lock the track on the side indicated by raised fist; travel signal with other hand
- Extend Boom (telescoping): Both fists in front of body, thumbs pointing outward
- Retract Boom: Both fists in front of body, thumbs pointing inward
Operational Signals
- Use Main Hoist: Tap fist on top of head, then use regular signals
- Use Whipline (auxiliary): Tap elbow with one hand, then use regular signals
- Dog Everything (pause): Clasp hands in front of body
- Move Slowly: Use one hand to give any motion signal, place other hand motionless in front of the signaling hand (the “modifier” hand indicates slow speed)
Voice and Electronic Communication
When hand signals aren't practical — such as on large sites, night operations, or when the signal person must be positioned far from the operator's line of sight — voice or electronic communication is used.
Two-Way Radio Protocols
Under 29 CFR 1926.1420, when voice signals are used, the signal person and operator must use a dedicated channel or frequency. The signaling method must be agreed upon before the lift, and the signal person must be clearly identifiable to the operator. Best practices include:
- Dedicated frequency: Never share the crane radio channel with general site communications
- Positive identification: Signal person identifies themselves before each sequence of commands
- Standardized terminology: Use consistent terms — “boom up,” “load down,” “swing left” — matching hand signal names
- Repeat-back confirmation: Operator repeats each command before executing
- Dead-man protocol: If communication is lost, all operations stop immediately
Camera Systems
Some modern crane operations use camera systems (boom cameras, load cameras, anti-collision cameras) to supplement signal person communications. However, under current OSHA interpretation, cameras do not replace the signal person requirement. Cameras are aids, not substitutes. If the operator cannot directly see the point of operation, a qualified signal person is still required even with cameras present.
Signal Person Positioning and Safety
A signal person who can't be seen is useless. One who positions themselves in the fall zone is in danger. OSHA's conduct standards for signal persons under 29 CFR 1926.1421-1926.1422 establish requirements for positioning:
- Visible to operator: The signal person must be positioned where the operator can clearly see the signals at all times during the lift
- Clear view of load: The signal person must be able to see the load, rigging, and the landing zone
- Outside the fall zone: Never position directly under the load or within the potential fall radius
- High-visibility clothing: OSHA requires signal persons to wear a high-visibility vest or garment — this isn't optional and applies at all times, including night operations (with retroreflective material)
- Single signal person per operator: Unless a clearly defined hand-off procedure is in place, only one signal person communicates with the operator at a time to prevent conflicting signals
The One-Signal-Person Rule
Multiple signal persons on a single lift create confusion and increase the risk of conflicting commands. OSHA addresses this in 1926.1419: only one signal person at a time gives signals to the operator. If a hand-off between signal persons is necessary (for example, as a load travels from one area to another), a clear protocol must be established. The outgoing signal person gives a “dog everything” signal, verbally confirms the hand-off, and the incoming signal person identifies themselves before resuming signals. The operator does not move the load during the hand-off.
Documentation Requirements
OSHA expects documentation of signal person qualifications, and smart crane companies maintain thorough records:
- Qualification records: Certification card (NCCCO or equivalent) or employer evaluation records showing the evaluator's qualifications, evaluation date, methods tested, and results
- Signal method selection: Pre-lift planning should document which signal method (hand, voice, electronic) will be used and any non-standard signals agreed upon
- Lift plans: On critical lifts, the signal person's position, communication method, and hand-off procedures should be documented in the lift plan
- Incident reports: Any signal-related near-misses or incidents should be documented and used for retraining
Common Signal Person Violations
Based on OSHA citation data, the most frequent signal person violations on crane operations include:
- No signal person present on blind lifts: The most common violation. Operators attempt lifts without a signal person when the load is out of their direct view.
- Signal person not qualified: Someone on the ground is waving their arms, but they have no formal qualification — no NCCCO certification, no employer evaluation, no documentation.
- Non-standard signals without pre-arrangement: Improvised hand gestures that the operator may misinterpret. One wrong signal during a steel set can result in a dropped load or a struck-by incident.
- No high-visibility clothing: Signal person wearing a standard hard hat and dark work clothes, blending into the background against earth-toned construction sites.
- Signal person in the fall zone: Positioning directly under the load path to get a “better view” — a fatal decision if rigging fails or the load shifts.
Training Program Best Practices
Whether you pursue third-party certification or employer qualification, an effective signal person training program should cover:
- Signal knowledge: All ASME B30.5 standard hand signals, demonstrated and memorized
- Crane operations basics: Load dynamics, boom deflection, swing, and how signals translate to crane movements
- Hazard recognition: Power lines, pinch points, fall zones, and unstable loads
- Site positioning: Where to stand, how to maintain visibility, escape routes
- Communication protocols: Radio procedures, hand-off procedures, emergency stop authority
- Practical assessment: Live signaling with an operator, demonstrating competency under realistic conditions
